9 Other Emergencies

• You may need to advise the patient how to push. When the contraction approaches its peak, the patient puts her chin on her chest, takes a deep breath, and begins to push as long and hard as she can while holding her breath. • Have her wait until the contraction peaks, then push for as long and hard as she can. If the contraction is long enough, the patient may push twice. • As the baby begins to emerge, get ready to support it and guide it out. Be prepared for the baby to emerge quite suddenly, even if initially it appears slowly. Usually the head is the first part to emerge. • Advise the patient not to push down while the head and neck are passing. One method to control involuntary pushing is through short quick breaths, or panting.

• Once the baby’s face and neck are clear of the vaginal opening, the patient may resume pushing with the contractions. Support the baby’s head in your palm and keep the neck as straight as you can.
