First Aid Documentation When rendering first aid to a patient, it is important to take notes on what you found and the treatment rendered. There are many guidelines on what information you should document while maintaining patient confidentiality. Guideline examples include: • Date and time of incident • Name of the patient • Date of birth • Gender • Signs and symptoms • Allergies • Medications • Past Medical History • Last Ate • Events leading up to injury or illness • What your assessment revealed • Treatment rendered and time of treatment • Time of arrival for paramedics
