Vital Signs LOC, Respirations, Pulse and Skin Condition Vital signs show the basic condition of your patient. A basic set of vital signs consist of the patient’s level of consciousness, their breathing rate per minute, their pulse or heart rate per minute, and their skin condition. Vital signs are “dynamic” and therefore can change from moment to moment. Once you have obtained the first set of vital signs you have a baseline to measure the patient’s developing condition. A set of vital signs should be obtained every 5 to 10 minutes or sooner if you notice any change in the patient’s condition.
