Premergency First Aid, CPR and AED Manual
2 PREmergency Scene Management
Hand-off and Documentation Hand-off to the next level of care is dependent on the patient’s condition. For a minor injury or ailment there may be nothing else to do after your involvement and there may have been no need to record the patient’s history (SAMPLE) or vital signs. For patients that require more care, you will have more information to pass on. Whether another first aider with more training than you, paramedics and firefighters or hospital staff, it is important you provide the next person who cares for the patient with this information at least verbally. Ideally you will be able to provide some record of the patient’s time in your care, whether in shorthand or a prepared form. In some cases, such as a workplace, you may have to provide your name and additional information for health and safety requirements.
